Special Supplemental Benefits for the Chronically Ill (SSBCI)

If you are a member of a Mass Advantage HMO plan and have a qualifying chronic condition, you may be eligible for extra benefits through Special Supplemental Benefits for the Chronically Ill (SSBCI). What are Supplemental Benefits for the Chronically Ill (SSBCI)?

Special Supplemental Benefits for the Chronically Ill are extra benefits available to certain Medicare Advantage members who have qualifying chronic health conditions. These benefits are designed to help improve or maintain your health and support your day-to-day needs. 

What are the qualifying conditions to be eligible for SSBCI benefits?

  • Autoimmune disorders
  • Cancer
  • Cardiovascular disorders
  • Chronic alcohol and other drug dependence
  • Chronic and disabling mental health conditions
  • Chronic heart failure
  • Chronic lung disorders
  • Dementia
  • Diabetes
  • End-stage liver disease
  • End-stage renal disease (ESRD)
  • HIV/AIDS
  • Neurologic disorders
  • Severe hematologic disorders
  • Stroke

Mass Advantage must receive information showing that you have at least one of the listed chronic conditions and that it meets all of the following criteria:

  • Is life threatening or significantly limits your overall health or function
  • You are at a high-risk of hospitalization or other adverse health outcomes
  • Requires intensive care coordination

How do I get approved for SSBCI benefits?

To be approved, Mass Advantage must have one of the following:

  • A completed attestation form from your treating provider confirming that you meet the SSBCI requirements; or
  • Medical claims that show you have an active, qualifying chronic condition and met the SSBCI requirements

Once all eligibility criteria has been met, your Prepaid Benefits Card will be updated with your Healthy Food & Produce Allowance* within 5-7 business days.

*The Healthy Food & Produce benefit are part of a special supplemental program designed for individuals with chronic illnesses. Eligible conditions include chronic alcohol and other drug dependence, autoimmune disorders, cancer, cardiovascular disorders, chronic heart failure, dementia, diabetes, end-stage liver disease, end-stage renal disease (ESRD), severe hematologic disorders, HIV/AIDS, chronic lung disorders, chronic and disabling mental health conditions, neurologic disorders, and stroke.
